Job summary

Houston Heights Nursing and Rehabilitation Center is seeking a licensed Physical Therapist for PRN coverage. The PT will conduct assessments and design therapeutic programs to improve mobility, strength, balance and endurance, including gait training and ROM testing.

The role requires collaboration with the Rehab Director, documentation of patient progress, and involvement in case conferences to promote positive outcomes. Texas licensure is required and home health experience is preferred.

Qualifications

• Graduate of a Physical Therapy program accredited by the American Physical Therapy Association or equivalent.
• Current Texas PT license to practice.
• Two years of PT experience; home health experience preferred.

Responsibilities

• Assess and reassess the patient’s level of function using diagnostic and prognostic procedures.
• Plan, implement and supervise an individualized therapeutic program to restore function, ROM, strength, balance and endurance.
• Coordinate with the Rehab Director to integrate the therapy plan into the overall care plan.
• Instruct patients in exercises and precautions to improve ROM, strength, balance and endurance.
• Observe, document and report patient responses and changes in condition and treatment.
• Educate family and agency staff on PT techniques and precautions; participate in case conferences.
