Job summary

ChenMed LLC is seeking a primary care physician who will deliver comprehensive care to older adults within a patient-centered model. The role emphasizes accountability for outcomes, strong care coordination, and collaboration with care teams to improve health and reduce costs.

The physician will manage acute and chronic conditions, supervise a care team, and engage with hospital partners as needed. Fluency in English and willingness to travel for training are required.

Responsibilities

• Engages with hospitalists for hospitalized patients and coordinates care.
• Responsible for assessment, diagnosis, treatment, management, and documentation for complex conditions.
• Leads a care team and coordinates resources for office-based patients.
