Job summary

Advocate Health is seeking a qualified Physical Therapist to perform comprehensive evaluations, create and implement individualized treatment plans, and coordinate discharge and home programs for patients. You will document care, manage daily tasks, ensure compliance with billing and department policies, and collaborate with the care team to optimize patient outcomes.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Maintains continuing education requirements as mandated by state standards.
• Physical Therapy License in the State of Illinois or State of Wisconsin; BLS certification.
• No experience required.
• Excellent communication, interpersonal, and organization skills.
• Intermediate computer skills.
• Ability to provide care appropriate to age of patients and demonstrate growth and development knowledge.

Responsibilities

• Interprets physician referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes and implements plan of care; monitors outcomes and adjusts treatment as needed.
• Completes discharge planning with home programs and family teaching; coordinates referrals.
• Documents per policy and manages billing, scheduling, and productivity requirements.
• May supervise PT assistants and rehab aides and supervise students as applicable.
• Engages in performance improvement activities to enhance patient outcomes.
