Job summary

Lebanon Rehabilitation is seeking an Occupational Therapist to assess referred patients in a long-term care setting and develop individualized rehabilitation plans under physician direction.

The OT will provide direct care, document progress, educate patients and families, and collaborate with certified occupational therapy assistants to maximize patient outcomes. A current state OT license and CPR certification are required; six months of long-term care experience is preferred.

Qualifications

• Graduated from an accredited school of occupational therapy.
• Current state license as an Occupational Therapist.
• Current CPR certification.
• Six months experience in a long term care environment preferred.

Responsibilities

• Assess referred patients and develop rehabilitation plans.
• Provide direct patient care and implement treatment programs.
• Document patient progress and communicate with families and staff.
• Refer patients to COTA when appropriate and supervise COTAs.
• Coordinate with the interdisciplinary team and participate in care conferences.
