Carolinas Medical Center in North Carolina is seeking a Telehealth/Care Coordination RN to join the Atrium Health Mobile Medicine - Care Coordination Center. The role involves applying the nursing process, coordinating care across teams, and maintaining safety across the continuum of care.

Formación

• Graduate of a Board of Nursing approved nursing education program.
• Active RN license with multi-state compact and/or state privileges.
• Basic Life Support (BLS) certification required; telehealth/licensing per state.
• Licensure for telehealth across designated service areas as applicable.

Responsabilidades

• Engages in evidence-based care and quality initiatives to improve outcomes.
• Uses the nursing process to assess, plan, implement, and evaluate care.
• Monitors patient conditions, adjusts plans, and collaborates with the care team.
• Maintains accurate EHR documentation and safety culture.
• May float to other units as needed to meet patient care needs.
• Communicates effectively with patients, families, and team members.
