Job summary

Emerus Holdings, Inc. is seeking a admissions coordination professional to support the admission and transfer process across our hospital network.

You will coordinate hospital admissions and transfers, and communicate with a multidisciplinary team to ensure continuity of care. This role emphasizes patient-centered service, documentation accuracy, regulatory compliance, and the ability to manage multiple tasks in a fast-paced environment.

Qualifications

  • Two years hospital-related or ER experience.
  • LVN or EMT certification required.
  • Healthcare transfer center experience preferred.
  • Must be fluent in English (written and spoken).

Responsibilities

  • Coordinate hospital admissions and transfers.
  • Facilitate conference calls between providers to coordinate continuity of care.
  • Identify and elevate issues, process deviations and service failures to leadership.
  • Maintain regulatory compliance with CMS, EMTALA, DNV, and HIPAA.
  • Maintain detailed documentation of admissions, transfers, conference calls, and related activities.
  • Communicate with multidisciplinary teams including providers, nurses, administrators on-call, hospital leadership, partner facilities, and non-partner facilities.
  • Establish good rapport and cooperative working relationships with transfer centers and referral sources.
  • Maintain a positive, professional, and collaborative demeanor in all interactions.
  • Provide superior customer service by listening, communicating empathetically and effectively.
  • Adapt to change with a willing and innovative spirit.
  • Meet individual and departmental goals and quality standards as assigned.
  • Safeguard sensitive and confidential patient PHI in remote work environment.
  • Apply clinical knowledge to gather and report appropriate clinical data for continuity of care.
  • Perform other duties as assigned.

The purpose of this position is to provide coordination of care during the admission and transfer process. The goal of the position is to enhance the quality of patient care through innovative and cost-effective best practices. This position promotes multidisciplinary integration to ensure successful continuity of care.

  • Responsible for coordinating hospital admissions and transfers
  • Facilitates conference calls between providers to coordinate continuity of care
  • Identify, resolve, and elevate issues, process deviations, and service failures to leadership
  • Maintain regulatory compliance with all state and federal regulatory agencies including, but not limited to, CMS Conditions of Participation, EMTALA, DNV, and HIPAA
  • Adhere to all company and departmental values, policies, processes, and procedures
  • Maintain detailed and accurate documentation of all admissions, transfers, conference calls, and all other activities related to essential job functions
  • Communicates accurately and timely with a multi-disciplinary team including but not limited to providers, nurses, administrators on-call, hospital leadership, partner facilities, and non-partner facilities
  • Establish good rapport and cooperative working relationships with partner transfer centers, non-partner transfer centers, and all other referral sources
  • Maintain a positive, professional, and collaborative demeanor in all interactions (oral, telephone, written, and electronic)
  • Provide superior customer service by listening, communicating empathetically and effectively in all interactions (oral, telephone, written, and electronic)
  • Adapt to change with a willing and innovative spirit
  • Meets all individual and departmental goals and quality standards as assigned
  • Safeguard sensitive and confidential company information including patient PHI in remote work environment
  • Apply clinical knowledge derived from education and experience as a foundation for gathering and reporting appropriate clinical data to ensure continuity of care
  • Perform other duties as assigned
  • 2 years hospital related or Emergency Room experience, required
  • LVN or EMT, required
  • Healthcare transfer center experience, preferred
