Transfer Center Coordinator RN – part‑time night shift, 379 Campus Dr, Somerset, NJ 08873.
Department: Patient Transfer Center. Pay Range: $64,700 – $84,300 per year.

Job Overview

This role provides comprehensive care coordination for patients, including initiation and coordination of incoming and outgoing transfers for higher levels of care. The Coordinator secures physician specialist acceptance, performs clinical assessment and triage, arranges appropriate transport resources, and oversees registration, bed placement, insurance authorizations, and patient transportation. The Coordinator ensures safe patient transfers according to established operational processes and guidelines for all service lines.

Responsibilities
  • Coordinate patient placement among multiple network facilities for admissions and transfers.
  • Prioritize patient transfers based on acuity.
  • Secure physician specialist acceptance and conduct clinical assessments.
  • Coordinate patient registration, bed placement requests, insurance authorizations, and transportation.
  • Assist activities such as patient registration, bed placement requests, insurance authorizations and arranging patient transportation.
  • Coordinate safe patient transfers according to established operational processes and guidelines for all service lines.
  • Alert staff at appropriate entity/agency of incoming/outgoing patients, schedule beds and services, perform clinical and financial screening of referred patients, and coordinate transportation.
  • Serve as a liaison with other departments, leaders and presiding physicians regarding process improvement opportunities for patient flow.
  • Manage individual patients across the healthcare continuum to achieve optimal clinical, financial, operational and satisfaction outcomes.
  • Maintain process for assessing current referral patterns for selected target areas and seek opportunities to increase business within assigned communities.
  • Collect and communicate pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
Scheduling Requirements
  • Submit 10 shifts of availability per calendar quarter in the scheduling portal by the 15th of the previous month.
  • Work 6 of the 10 shifts submitted.
  • Shifts are 12 hours long.
Essential Functions
  • Evaluate medical necessity and appropriateness of care, optimizing patient outcomes.
  • Assess patient admissions and continued stay using standard criteria.
  • Facilitate patient throughput to a final destination facility with focus on quality and efficiency.
  • Coordinate patient placement among multiple outreach and network facilities.
  • Serve as a liaison with other departments, leaders, and presiding physicians regarding process improvement.
  • Maintain compliance with regulations such as EMTALA, HIPAA, and quality initiatives.
Qualifications – Required
  • Active BSN RN license in the state of practice, a temporary RN license, or compact RN licensure for the current state of practice.
  • Critical Care Experience (ALS/PALS/BLS) with at least three years of experience in the care of the population served.
  • Experience in clinical care operations, case management, or patient throughput.
  • Working knowledge of hospital operations, medical/nursing staff procedures, and community resources.
  • Critical thinking, conflict resolution, time management, and problem‑solving skills.
  • Excellent communication skills and ability to interact well across departments, facilities, and organizations.
  • Excellent organizational and human relations skills to maintain effective relationships with internal and external customers.
Qualifications – Preferred
  • Expert level of Microsoft Office.
  • Bachelor’s degree in nursing or related field.
  • Additional related education and/or experience.
  • Typing speed of at least 40 wpm.
Certifications and Licenses Required
  • Active BSN RN license in state of practice, temporary RN license, or compact RN licensure for current state of practice.
