Job summary

Hackensack Meridian Health is hiring for a transfer center nurse within a busy acute care setting in New Jersey. You will handle inter-facility communications, documentation in the electronic health record, and collaborate with EMS and hospital teams to assure appropriate care levels and timely transport.

The role requires an RN with at least 2 years in acute care, with related critical care/EMS experience and strong communication skills.

Qualifications

  • RN graduate of an accredited school of nursing.
  • Minimum of 2 years clinical experience in acute care setting.
  • BSN preferred and transfer center experience if available.
  • Critical care and EMS experience recommended.
  • Computer literacy and medical terminology required.
  • Excellent communication skills; able to multitask.
  • Flexible and adaptable to change; works well under pressure.

Responsibilities

  • Answers telephones per established policies; maintains professional tone.
  • Coordinates communication between sending and receiving facilities and within the receiving facility.
  • Records details of transfer requests in the electronic health record promptly.
  • Collaborates with healthcare team to ensure appropriate level of care and room assignment.
  • Monitors bed availability across units involved in transfers.
  • Escalates barriers following policies and chain of command.
  • Assists with patient identification and interfaces with patient access as needed.
  • Maintains a clean, sanitary work environment and participates in onboarding of new staff.

Responsibilties
  • Answers telephones in accordance with established policies and procedures.
  • Utilizes professional tone and demeanor in all communication.
  • Coordinates communication between sending and receiving facilities, and within the receiving facility.
  • Accurately records details of all communication related to each transfer request in the electronic health record in an efficient and timely manner.
  • Works in a collaborative manner with other members of the health care team.
  • Utilizes clinical expertise, and collaborates with capacity management, nursing, physician/provider representatives, and other hospital departments to assure proper level of care and room assignment at the receiving facility.
  • Maintains awareness of bed availability on all patient care units and outpatient departments involved in the inter-facility transfer and direct admission of patients.
  • Demonstrates the ability to handle a high degree of pressure, heavy workloads, multiple requests, and numerous interruptions in a positive manner, establishing priorities for effective work completion. Able to diffuse challenging situations and respond appropriately.
  • Demonstrates expertise in working with multiple computer applications as well as all functions of the telephone system.
  • Demonstrates the ability to independently implement solutions to transfer center issues.
  • Escalates barriers and concerns using applicable policies and procedures as well as chain of command.
  • Follows patient identification policies and procedures to ensure accurate patient identification. Collaborates with patient access services as needed.
  • Utilizes downtime procedures for effective communication and documentation when necessary.
  • Collaborates with Emergency Medical Services (EMS) to arrange appropriate and timely transportation based on established protocols.
  • Collaborates with sending and receiving providers to ensure appropriate level of care and response urgency are assigned for patient transportation.
  • Obtains insurance information and communicates with insurance providers and/or case management and social services as needed.
  • Initiates registration activities and completes registration as requested.
  • Prepares handoff and other reports as directed.
  • Mentors team members. Participates in onboarding of new team members.
  • Maintains a clean and sanitary work environment.
  • Other duties as assigned.
  • Patient age specification: Neonate, infant, toddler, pre-school, school age, adolescent, adult, geriatrics.
  • Must be able to sit and work at a computer with multiple monitors while speaking on a telephone for extended periods of time.
Qualifications

Education, Knowledge, Skills and Abilities Required:

  • RN graduate of an accredited school of nursing.
