Job summary

The San Diego LGBTQ Community Center is seeking a full-time Client Support Specialist to provide case management and connect gender-diverse clients with services across San Diego County. You will develop individualized care plans and collaborate with internal teams and partners to remove barriers faced by transgender, nonbinary, and gender expansive individuals.

Qualifications

• 1 year of experience providing case management to transgender, nonbinary, and gender expansive communities.
• Fluency in English required.
• Proficient with Microsoft Office in PC/Windows environments.
• Demonstrated ability to manage multiple tasks with shifting priorities.
• Competence in working with diverse populations and using trauma-informed approaches.

Responsibilities

• Provide case management by creating individualized care plans to accomplish goals.
• Provide linkages to gender-affirming care and referrals in collaboration with internal and external partners.
• Navigate health insurance and assist with grievances and appeals, including prior authorizations where needed.
• Assist clients in securing employment, financial security, and housing benefits.
• Attend quarterly discussions to ensure accurate demographic information.
• Facilitate the Project TRANS Advisory Committee and related activities.
• Submit monthly program impact stories and maintain program data.
• Support administrative duties for the Project TRANS program and communications.
