Job summary

The San Diego LGBT Community Center is hiring for a transgender and gender expansive program role. The position focuses on peer-to-peer advocacy to engage clients with services across health, housing, immigration, and benefits, while protecting privacy and promoting equity.

On-site work in San Diego with some evening and weekend events; strong collaboration with multiple departments and community partners.

Qualifications

• 1 year of experience providing case management services to transgender, nonbinary, and gender expansive communities.
• Fluency in English required.
• Proficient with Microsoft Office; comfortable with PC/Windows.
• Ability to manage multiple tasks with shifting priorities; experience with diverse populations; trauma-informed approach.

Responsibilities

• Provide case management by creating individualized care plans to accomplish goals.
• Provide linkages and referrals to gender-affirming care in collaboration with internal Center departments.
• Navigate health insurance and assist with grievances and appeals.
• Assist clients in securing employment, financial stability, and housing resources.
• Attend quarterly discussions to ensure accurate client demographics and contribute to program impact stories.
• Facilitate support groups and advisory committee activities.
• Assist in administrative duties and program calendar planning.
