Job summary

UC San Diego Health seeks a Trans Health Project Case Manager to deliver trauma-informed, gender-affirming case management for Black and Latina transgender women within the THP initiative. You will coordinate care, link to resources, and support engagement with services while ensuring confidentiality and research compliance.

Ideal candidates have a social work degree, experience with LGBTQ communities, and a commitment to equity.

Qualifications

• Bachelor's degree in social work from a CSWE-accredited program with 1 year of experience, or equivalent.
• Proficient with Microsoft Office; comfortable in PC/Windows environments.
• Ability to manage multiple tasks with shifting priorities.
• Experience with LGBTQ communities and transgender women in San Diego County.
• Fluent in English.
• Ability to work with diverse populations.
• Knowledge of person-first and inclusive language used with minority populations.

Responsibilities

• Provide comprehensive, trauma-informed, gender-affirming case management support.
• Support participants in identifying goals, advocating across care systems, and accessing gender-affirming services.
• Provide care coordination, resource linkage, and service advocacy with community partners and researchers.
