Qualifikationen

• BSN in Nursing and at least 3 years in a clinical setting.
• Current licensure in Massachusetts as a Registered Nurse.
• Preferred experience in an office setting and/or with surgical care management.

Aufgaben

• Serve as the primary clinical contact for Total Joint Program patients across the surgical continuum.
• Provide pre- and post-operative education to patients and caregivers, including preparation, recovery expectations, Rehab, medication management, and discharge planning.
• Coordinate care and facilitate communication among surgeons, physical therapy, care management, home health, and other interdisciplinary team members.
• Conduct telephone triage and post-operative follow-up, reinforce care plans, and address barriers to recovery.
• Collect and report Total Joint Program quality and outcomes data.
