Qualifications

• Connecticut RN license required.

Responsibilities

• Collaborate with Service line leadership to coordinate comprehensive risk assessments for TKA/THA patients and enroll high-risk patients into risk modification programs.
• Advise on planning, discharge, and transition of care, facilitating communication between patients, families, physicians, and care teams.
• Maintain documentation in EMR, track patient flow, and support post-discharge follow-up and patient education.
