Job summary

PruittHealth is seeking a Physical Therapist for Home Health Services in Griffin, GA. The role involves evaluating patients in their residence, providing evaluative, consultative, and therapeutic interventions, and ensuring effective communication with families and care teams.

The incumbent will document progress, maintain licensure, and collaborate with the physician to manage the patient care plan. A strong emphasis on home-based care and adherence to regulatory standards is required.

Qualifications

• Must have a valid PT license in the state of practice.
• Graduate of an accredited Physical Therapy Program.
• Foreign-trained PT must have credentials evaluation showing substantial equivalence.

Responsibilities

• Evaluate function and skilled therapy needs.
• Treat to relieve pain and improve function per plan of care.
• Collaborate with family and interdisciplinary team.
• Document progress using required forms (OASIS, SOC/Admissions, etc.).
• Maintain care plan with physician and team, referring as needed.
• Complete timely documentation at point of care.
• Meet productivity standards and participate in required trainings.
