Job summary

Qualifications

• Must have a valid physical therapy license in the state of practice.
• Graduate of an accredited Physical Therapy program.
• If foreign-trained, the program must be deemed substantially equivalent to U.S. PT entry-level education.

Responsibilities

• Evaluate patient function and determine skilled therapy needs.
• Provide therapy to relieve pain and restore function per the plan of care.
• Communicate with family and interdisciplinary team; document progress timely.
• Conduct home visits and monitor outcomes using required documentation.
• Coordinate care, make referrals as needed, and maintain regulatory compliance.
