Job summary

PruittHealth Corporation is seeking a Licensed Practical Nurse (LPN) for Home Health to provide skilled nursing care under RN supervision. This role emphasizes a new pay-per-point model, enabling higher pay for visits demanding more effort and time outside standard visits.

Qualified candidates hold an active LPN license in the state, have at least 1 year of nursing experience, and are prepared to perform home visits with required documentation and safety practices.

Qualifications

• Nursing degree from an accredited college or university, or graduate of an accredited/approved LPN program.
• Minimum of one (1) year of practical nursing experience
• Current and unrestricted Licensed Practical Nurse (LPN) licensure in state of practice.
• Liability insurance to complete home visits

Responsibilities

• Provides basic nursing care and interventions to home health patients under RN supervision.
• Monitors, documents and reports patient health progress using required documentation.
• Communicates changes with RN supervisors and care team.
• Completes documentation timely, accurately, at the point of care.
• Meets agency productivity standards and uses time/resources efficiently.
• Participates in surveys, in-services, case conferences, staff meetings, and programs.
• Follows safety regulations and reports occupational hazards as necessary.
• Maintains current license, certification and clinical records per regulations.
