Job summary

PruittHealth is seeking a Licensed Practical Nurse (LPN) for Home Health Services to provide essential nursing care in patients' homes under RN supervision. The role offers a new pay-per-point model with top-tier pay for higher-effort visits and a comprehensive benefits plan, including mileage reimbursement.

Qualified candidates will have a valid LPN license, at least 1 year of nursing experience, and the ability to work independently while coordinating with the care team.

Qualifications

• Nursing degree or LPN program graduate.
• Minimum of one year of practical nursing experience.
• Current and unrestricted LPN licensure in state of practice.
• Liability insurance to complete home visits.

Responsibilities

• Provides basic nursing care and interventions to home health patients under RN supervision.
• Monitors, documents and reports patient health progress using required forms.
• Communicates changes to RN supervisors and care team as appropriate.
• Completes timely documentation at the point of care.
• Maintains safety and meets agency regulations and guidelines.
