Job summary

PruittHealth is seeking a Licensed Practical Nurse (LPN) for Home Health Services in a full-time role. You will provide basic skilled nursing care under RN supervision, with a compensation plan that ties pay to visits and effort, including mileage reimbursement and flexible schedules.

Minimum qualifications include an LPN license in the state of practice, at least one year of nursing experience, and a nursing degree from an accredited program.

Qualifications

• Nursing degree from an accredited college or university, or a graduate of an accredited/approved LPN program.
• Minimum of one year of practical nursing experience.
• Current and unrestricted LPN licensure in state of practice, with liability insurance for home visits.

Responsibilities

• Provides basic nursing care and interventions to home health patients according to state and industry best practices.
• Demonstrates basic knowledge of home care and competency in nursing for patient care skills.
• Monitors, documents and reports patient health progression using required documentation and timelines.
• Communicates with RN supervisors and patient care team about changes noted during care.
• Completes documentation timely and accurately at the point of care according to industry standards.
