PruittHealth is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under RN supervision. You will document progress, coordinate with the care team, and perform routine visits following state requirements and agency guidelines.

This role emphasizes travel to patient homes, strong clinical skills, and adherence to safety and documentation standards. A current GA LPN license and liability insurance are required, with prior home health experience preferred.

Qualifications

• LPN with nursing degree from an accredited program.
• Current and unrestricted LPN licensure in the state of practice.
• Minimum of one year of practical nursing experience.
• Liability insurance for home visits.
• Home health or hospice experience is preferred.

Responsabilités

• Provides basic nursing care and interventions to home health patients, per state and industry practices.
• Demonstrates home care knowledge and competency for patient care skills.
• Monitors, documents and reports patient progress using required documents and tech.
• Communicates with R N supervisors and care team about changes noted during care.
• Completes documentation timely and accurately at the point of care.
• Meets agency productivity standards and uses time/resources efficiently.
• Participates in surveys, in-services, case conferences, meetings, and programs as required.
• Follows safety regulations and reports occupational hazards as needed.
• Maintains current license, certification, and clinical records per regulations.
• Is self-directed, prioritizes tasks, and meets deadlines.
