AlohaCare seeks a TOC Supervisor to lead the transitions of care team, coordinating post-discharge activities and ensuring safe, timely, and effective transitions for members in Hawaii. You will help reduce readmissions and improve post-discharge outcomes through education, referrals, and follow-up.

The role collaborates with Health Coordination, Medical Management, Behavioral Health, and Quality teams, supervising staff, ensuring compliance with QUEST Integration, Medicare DSNP, and CMS

Qualifications

• Licensed RN, LCSW or LSW in the State of Hawaii.
• Minimum of 3 years experience in a health plan or clinical setting.
• Minimum of 1 year experience managing and developing a team.
• Prior experience in clinical assessment, discharge planning, and utilization management.
• Able to work in a fast-paced, matrixed environment with multiple priorities.

Responsabilités

• Supervises TOC team and day-to-day operations of the Transitions of Care Program.
• Supports TOC workflows aligned with QUEST Integration, CMS Model of Care, NCQA Care Transitions requirements.
• Provides coaching, mentoring, and training to TOC staff.
• Serves as TOC subject matter expert for staff and partners.
• Assists with workflow design, education, and performance monitoring.
