Job summary

BayCare Health System is seeking a TOC Clinical Pharmacist at their Clearwater office to provide outstanding customer service and support patient discharge follow-up. The ideal candidate will engage in patient care and medication safety to reduce hospital readmissions.

Qualifications include a Bachelor's Degree, with a PharmD preferred, and a valid Florida pharmacist license. The role offers medical benefits, paid time off, and career growth opportunities within the largest employer in the Tampa Bay area.

Qualifications

• Current Pharmacist license in the State of Florida.
• Preferred ASHP certification as a Pharmacist.
• Experience in a clinical pharmacy setting is preferred.

Clinical Pharmacist Responsibilities

The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.

• Facility: St. Joseph's Hospital
• Location: Tampa, FL
• Status: PRN as needed
• Shift: 8:00AM - 4:30PM
• Weekend Work: None
• On Call: No
