Job summary

American Academy of Family Physicians seeks a School Based APC- FNP at Tioga Junior High School to provide primary care to students under the supervision of the SHC Director. The role centers on adolescent and pediatric care within predefined policies and clinical protocols.

The advanced practice clinician works with the Medical Director to review health records, arrange referrals, and ensure documentation meets regulatory standards.

Qualifications

• Current licensure through the LA State Board of Nursing or eligibility to obtain prior to employment.
• Excellent written and verbal communication skills; bilingual (English/Spanish/French) preferred.
• Valid DEA license or ability to obtain prior to employment.
• BLS certification obtained by the American Heart Association.

Responsibilities

• Provide primary care to adolescents and children within school-basedclinics under supervision.
• Collaborate with the Medical Director and health team in assessment, diagnosis, and treatment.
• Develop, implement, and review health center policies and patient care plans.
• Maintain accurate medical records and coordinate referrals to other providers.
• Educate patients and families on preventive care and treatment options.
