Job summary

Rising Ground, Inc. is seeking a Full Time Professional in Queens, NY to conduct intakes, assessments, ISP development and ongoing case management for RHY participants. The role reports to the Lead Social Worker and involves counseling, housing applications and collaboration with service providers.

Requires an MSW and licensure in social work (LMSW/LCSW preferred). The position offers benefits, remote/flexible options and a competitive salary.

Qualifications

Responsibilities

• Ensure quality counseling, case management and support is provided to all active participants.
• Provide weekly documented case notes for sessions held.
• Review treatment and ISP to ensure they meet the needs of the youth.
• Ensure paperwork for youth is completed in a timely fashion including intakes, case notes, Bio Psycho Socials/assessments, and referral material.
• Provide clinical counseling for those youth that display a need for more intense counseling or are assessed to have possible mental health needs on an as needed basis.
• Complete all necessary housing applications for participants as they transition from RHY programs.
• Participate in weekly Huddle and Personal Development meetings (PDMs) with Program managers and participants.
• Facilitate/ co-facilitate workshops/ groups on a weekly basis.
• Enter data accurately and timely in PTS and other designated systems.
• Liaison with existing service providers that provide support to youth.
• Track services and referrals for monthly and quarterly reports.
• All other relevant duties as delegated Conduct outreach and presentations to recruit youth to the Drop -In Centers or TIL programs.

Salary: $68,250.00 Annually

Overview

Under the supervision of the Lead Social Worker, the social worker will be responsible for conducting Intakes, mental health assessments, screenings, Individual Service Plans (ISP), case management and therapeutic services to program participants who access the Runaway and Homeless youth programs (RHY). The social worker is accountable for overseeing and implementing care and resources to meet the physical, emotional, educational, and social needs of the residents within the program.

Position Type/Expected Hours of Work:

This position is in a 24/7 program. Additionally, evening and weekend hours may be required.
