Job summary

Qualifications

The Case Manager, under general supervision performs a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to consumers and their families. Monitors consumer progress towards treatment goals. Extensive documentation is required; accurately charts services and consumer progress. Must be able to work well with community agencies and function as part of a team.

This position requires:

• Bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery.
• Knowledge of mental illness
• Valid Georgia driver’s license

Working Conditions:

Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations. May have extended periods of sitting, walking, and standing. Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in LBHS or personal vehicle.
