Responsibilities
- Coordinate regional deployment readiness and timelines for sites.
- Lead regional implementation and training across healthcare partners.
- Coordinate workflows among CHWs, supervisors, providers, and care teams.
- Coordinate program evaluation, data collection, and CQI activities.
- Facilitate regional stakeholder meetings and communications.
- Ensure alignment with statewide goals and share best practices.
