Job summary

Qualifications

• Master's degree in Social Work and CSW license required.
• Knowledge of Kentucky board laws and regulations.

Responsibilities

• Provide clinical services: intakes, group/individual/family therapy, crisis intervention.
• Maintain and complete service documentation (assessments, case notes, reports).
• Develop treatment plans and reviews based on client progress.
• Collaborate with CHR, school officials, family members, and health agencies.
• Participate in staff meetings, treatment planning meetings, case conferences and seminars.
• Write correspondences for clients and families (courts, letters, etc.).
• Present cases and consult with other agencies.
• Formulate clinical diagnoses for mental health and substance abuse.
• Create and follow clinical intervention plans.
