Job summary

Meridian HealthCare in Youngstown, Ohio, is seeking a licensed counselor to join an integrated primary care and behavioral health network. You will conduct assessments, develop treatment plans, and provide individual counseling to support patients in a holistic care model.

You will work with medical and behavioral health professionals, coordinate with care coordinators, and participate in weekly supervision to ensure high-quality patient care.

Qualifications

• Master's degree required in counseling, social work, or a related field with corresponding licensure credentials (LSW/LISW, LPC/LPCC).
• Competency or training in mental health, substance use disorder treatment, and co-occurring disorders.

Responsibilities

• Complete assessments and treatment planning.
• Provide individual counseling and patient access to individualized treatment.
• Participate in an integrated healthcare network to deliver high-quality patient care.
• Work side-by-side with medical and behavioral health professionals.
• Coordinate with care coordinators to provide wrap-around services.
• Engage in weekly individual supervision.
