The Association for Individual Development (AID) in Dekalb, IL seeks a Therapist Qualified Mental Health Professional (QMHP) to provide case management and counseling to adults and children. The role includes on-site and community/home-based services, with some evening hours and teamwork with other providers to meet client needs.

Requirements include a Master’s degree in a human services field and preferably licensure (LPC or LSW).

Responsibilities

We are looking for a Therapist Qualified Mental Health Professional for Dekalb, IL who exemplifies that mission and wants to work for an organization that makes a difference.Schedule: Tuesday-Friday 8-6:30pm, must be able to work 2 evenings per weekWhat will you be doing?Therapist QMHP are responsible for the provision of case management and counseling services to both adults and children/adolescents who are diagnosed with persistent mental illness. Services are provided in family and individual treatment settings with both adult and children/adolescents. Services are offered both onsite and, in the community/client home. Therapists are expected to work with a team of service providers within the agency and throughout the social service community to ensure that all needs of clients are met.Essential Job ResponsibilitiesAttendance at scheduled work hours including evening hoursProvide effective individual, couples, & family counseling, as assignedComplete clinical assessments that meet the requirements of program funders and applicable administrative rulesRefer clients for community-based case management services as neededProvide case management services as medically necessaryComplete required documentation (IM+CANS, documentation including encounters, consents) on a timely basisMeet minimum service provision/billing standards.Acquire and maintain required certifications and trainingsAbide by AID’s Code of Ethics, policies, and proceduresMaintain professional judgment and performance regardless of personal issuesEmployee promotes awareness and respect for the diversity of our clients, employees, families and other stakeholders.What will we provide Full Time employees.
