Job summary

Live Well Homecare is seeking an Outpatient Therapist to provide individual, group, and family therapy sessions addressing psychological, emotional, and substance use issues. You will conduct therapy, participate in treatment planning, and collaborate with physicians, nursing, and utilization review staff to optimize care.

The role emphasizes proactive communication with clients, families, and referral sources, along with thorough documentation and adherence to care plans.

Qualifications

• Master's degree in Social Work or Counseling and current Clinical License.
• Basic Life Support certification (within 30 days of hire).
• ACLS and PALS certifications (within six months of hire).
• Preferred: One year of relevant experience and ASLS certification.

Responsibilities

• Conduct therapy sessions and actively participate in treatment planning.
• Provide family counseling to ensure effective communication and support.
• Engage with clients, families, and referral sources through proactive communication.
• Collaborate with interdisciplinary team members including physicians, nursing, and utilization review staff.
• Communicate treatment progress and length of stay considerations to relevant team members.
