Job summary

LiveWell Homecare Agency, LLC is seeking an Outpatient Therapist to provide individual, group, and family therapy sessions focused on psychological, emotional, and substance abuse issues.

Responsibilities include conducting therapy, treatment planning, family counseling, collaboration with physicians, nursing, and utilization review staff, documenting progress, and ongoing professional growth through training and benefits.

Qualifications

• Master’s degree in Social Work or Counseling is required.
• Current Clinical License required.
• Basic Life Support certification within 30 days of hire.
• ACLS and PALS certifications within six months of hire.
• Preferred: One year of relevant experience and ASLS certification.

Responsibilities

• Conduct therapy sessions (individual, group, family) and participate in treatment planning.
• Provide family counseling to support communication and relationships.
• Engage with clients, families, and referral sources via proactive communication.
• Collaborate with interdisciplinary team members including physicians, nursing, and utilization review staff.
• Communicate treatment progress and length of stay considerations to team members.
• Complete accurate and timely documentation for clients.
• Perform additional duties as assigned.
