Responsibilities

• Monitor and deliver high-quality services to dually diagnosed/chemically dependent patients.
• Coordinate the full continuum of care; assess and document clinical needs.
• Implement and deliver individual, group and educational services; provide safe, therapeutic milieu.
• Crisis intervention and collaborate with the multidisciplinary team to meet treatment plan objectives.
• Discharge planning and referrals to community resources.
