Job summary

Personalized Health Partners is seeking Social Workers and Counselors to join as Therapist/Behavioral Health Care Managers. A Master’s degree and a state license (LSW/LISW, LPC, LPCC) are required to provide psychotherapy and group therapy to residents in need.

You will collaborate with psychiatrists and primary care teams, coordinate care, and document thoroughly in an EMR system. Positions offer flexible scheduling, with both full-time and part-time options to fit your career goals.

Qualifications

• Licensed social workers or counselors are required.
• MSW or MS in Counseling from an accredited program.
• Experience with diverse patient populations desired.

Responsibilities
