Job summary

Personalized Health Partners seeks Social Workers/Counselors to join as Therapist/Behavioral Health Care Managers. You will provide psychotherapy, group therapy, and collaborative care management to residents in need within long-term care settings.

Candidates must hold a Master’s degree and be licensed as LSW, LISW, LPC, or LPCC. The role emphasizes teamwork in a multidisciplinary environment, using EMR systems for documentation and care coordination.

Qualifications

• Must hold a Master’s degree in Social Work or Counseling.
• Licensed as LSW/LISW or LPC/LPCC (or LPCC-S/LISW-S) required.
• Experience with diverse patient populations and multidisciplinary teams preferred.
• Proficiency using electronic medical records (EMR) systems preferred.

Responsibilities

• Provide individual psychotherapy to residents.
• Deliver group psychotherapy sessions.
• Coordinate collaborative care management with psychiatrists and primary care providers.
• Conduct initial behavioral health evaluations when needed.
• Maintain detailed clinical documentation in EMR.
