Job summary

RFS Behavioral Health (TACKLE Program) seeks a licensed social worker or counselor to provide in-home or in-office therapy across schools, homes, and community settings. You will develop treatment plans, document goals and progress, and coordinate with families and school staff to support client outcomes.

The role requires at least one year of experience, strong time management and communication skills, and the ability to work a flexible schedule, including some evenings.

Responsibilities

Job description

Works with individuals, groups and communities to improvemental health and behavior. Provides therapy to clients in and out of theschool environment as well as in the home. Creates and updates diagnosticassessments, treatment plans and treatment plan reviews.

Essential Dutiesand Responsibilities

• Deliver in-home or in-office services to clientsas requested by the area mental health including the specific hours requested.Services may include going into the school.
• Provide therapeutic interventions to theidentified client in order to enhance and foster the client’s optimalfunctioning and interactions in the home and community.
• To continually assess client needs, service availability and appropriateness.
• Work cooperatively with treatment team membersand the family in the provision of services to meet the goals developed in thePerson Centered Plan.
• Provide appropriate documentation for servicedelivery including treatment goals, progress notes and billing and submit saiddocumentation as specified by Medicaid and CARF standards.
• Maintain required billable hours. Work minimumof one evening per week, with evenings being 10 am – 6pm or 11am – 7pm,discretion upon supervisor.
• Maintain productivity of 70% of worked hours
• Assist in creating and maintaining group therapysessions
• Complete and maintain all continuing educationand licensure
• Maintains quality results by following agencystandards.
• Maintains continuity of work operations bydocumenting and communicating actions, irregularities, and continuing needs.
• Serves and protects the agency by adhering toprofessional standards, agency policies and procedures, federal, state, andlocal requirements, and CARF standards.
• Perform other duties, as assigned, to the needof the agency deemed necessary and appropriate

(including but not limited to completion ofSocial Security forms, authorization or billing forms, phone calls, andparticipating in case consultation meetings with schools).
