Job summary

Topaz Fiscal Services is seeking an Activities Director for a Skilled Nursing Facility. You will plan and oversee a comprehensive activities program addressing residents' physical, cognitive, social, and emotional needs to promote well-being.

You will conduct assessments, ensure 7-day programming, manage staff and volunteers, and coordinate with the care team to address individual goals. This role requires strong regulatory knowledge and collaboration with families.

Qualifications

• High school diploma or GED required, associate's or bachelor's in related field preferred.
• Minimum of 2 years' activities programming in long-term care or senior living.
• ADC/ACC certification or ability to obtain within 6 months of hire.
• Knowledge of CMS F-tag requirements and regulations governing SNF activities.

Responsibilities

• Plan, implement, and evaluate a monthly activities calendar tailored to residents' interests and care plans.
• Conduct initial and ongoing resident activity assessments (MDS F) with care team.
• Facilitate group activities (arts, music, exercise, reminiscence, religious services, outings).
• Coordinate 1:1 activity visits for residents unable to participate in groups.
• Ensure programming is available 7 days a week, including evenings and weekends.
• Maintain CMS compliance and document in EMR; complete MDS F assessments timely.
• Develop individualized activity care plans with IDT; update per regulations.
• Recruit, train, supervise activities staff and volunteers; schedule coverage.
• Engage community organizations and families; lead special events and celebrations.
• Manage department budget and inventory; submit required reports; participate in QAPI initiatives.
