Job summary

Wesley Enhanced Living is seeking a Registered Nurse Assessment Coordinator (RNAC) for our Philadelphia facility. You will oversee federally mandated MDS-based assessments, formulate individualized nursing plans, and educate the interdisciplinary team on care planning and documentation.

Responsibilities include ensuring accurate MDS sections, communicating assessments to insurers, and leading joint care conferences with social workers.

Qualifications

• Knowledge of ICD-10 coding with attention to detail to ensure complete/accurate assessments.
• RN license active; CPR certification current; AANAC certification or within 12 months of employment.

• Follow CAA process to develop care plans with the treatment team.
• Manage clinical and financial records and oversee payor communications and appeals.

Position Summary:

The Registered Nurse Assessment Coordinator (RNAC) is responsible for federally mandated healthcare assessments of residents in a long‑term care facility and formulates individualized nursing plans of care. Implements, reviews, assesses, and educates the interdisciplinary team on the MDS and individualized care plans and performs other duties as required.

Essential Duties and Functions:

• Ensure accuracy of all sections of Multiple Data Set (MDS) and ensure accuracy of clinical documentation to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate resident care (CMI).
• Communicate resident assessment and condition to insurance companies to maximize resident care days in the community.
• Serve as a primary educator to the interdisciplinary team to ensure documentation is accurate, timely, and thorough.
• Take joint care conferences with social workers.
• Follows the assessment procedures outlined in the Centers for Medicare and Medicaid Services’ (CMS) Resident Assessment Instrument (RAI) to conduct comprehensive healthcare assessments upon admission of residents.
• Follows the Care Area Assessment (CAA) process to develop comprehensive nursing care plans for treatment of individual long‑term care residents based upon collected and coded MDS data in collaboration with interdisciplinary treatment team members.
• Manages clinical and financial record keeping and dissemination of these records to appropriate payors. This includes overseeing UMR reviews and any appeals processes.

Qualifications include:

• Knowledge / SME coding (ICD‑10), with attention to detail to ensure assessments are complete and accurate.

Education:

• Graduate of an accredited school of nursing.

Licensure/Certification:

• Current, active RN license; must maintain current CPR certification and be AANAC certified (or have within 12 months of employment).
