Job summary

Choice in Aging is seeking a Registered Nurse Care Manager (RNCM) to work with frail, low-income elders enrolled in the MSSP Medi-Cal Waiver. The RNCM will assess needs, coordinate services, and help clients maintain safe community living, within the CiA team.

This part-time position requires strong nursing skills, English proficiency, and a CA driver’s license. The role includes home-based assessments, care planning, and collaboration with MSSP and CiA staff to deliver holistic care to

Qualifications

• Bachelor’s Degree in Nursing required.
• 1-2 years of clinical nursing experience, preferably with frail older adults.
• California RN license required; public health certificate a plus.
• English communication required in written and verbal form.
• Valid California driver’s license, clean DMV record, and automobile insurance.
• Strong care management skills for vulnerable crisis-prone participants.
• Computer literacy and Excel proficiency.
• First aid/CPR certification.
• DOJ fingerprint clearance required.

Responsibilities

• Screen applicants to MSSP using established criteria and screening forms to determine level of care appropriate for MSSP.
• Conduct Initial Health Assessment – in participant’s home or via phone.
• Reassess the medical portion of the yearly reassessment in person or via phone.
• Reviews and completes initial MSSP contracts with participants (Application, Release of Information).
• Identify any/all medical-related need placing the participant at risk for injury or placement.
