Job summary

Choice in Aging is seeking a Registered Nurse Care Manager - Casual (RNCM) to support frail, low‑income elders enrolled in the MSSP Medi‑Cal Waiver. You will evaluate participants holistically, coordinate health and social services, and work within the CiA team to help clients age in place with dignity in community.

The RNCM counsels and coordinates care, conducts home or remote assessments, and ensures documentation and licensing requirements are met.

Qualifications

• 1–2 years of clinical nursing experience, preferably with frail older adults or 1 year of case management.
• Ability to communicate in English, written and verbal.
• Strong computer literacy to use electronic data management systems and Excel.
• Current first aid/CPR certification.
• Must pass Department of Justice fingerprint clearance.

Responsibilities

• Screen MSSP applicants using criteria and screening forms to determine level of care.
• Conduct Initial Health Assessment – in participant’s home or remotely.
• Reassess the medical portion of the yearly reassessment in person or remotely.
• Review initial MSSP contracts with participants (Application, Release of Information).
• Identify any medical needs placing the participant at risk for injury or placement.
