CareMed Health Solutions seeks a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities. The role involves managing acute changes in condition, advanced care planning, chronic care management, and prescribing medications in accordance with Kentucky law.

You will order and review treatments, communicate with nursing home staff, coordinate with the primary care physician, and work within an interdisciplinary team to deliver high-quality elder care.

Qualifications

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Basic coding knowledge preferred, not required.

Responsabilités

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.
