Job summary

Gateway Hudson Valley is seeking a Case Manager at our Kingston, NY corporate headquarters to oversee Home Delivered Meal and elderly services, ensuring high-quality care and helping participants navigate their needs. The role requires coordinating assessments, securing eligible benefits, documenting NYSOFA and OAA forms, and leading outreach within the Elderly Program.

Strong knowledge of senior resources and effective care coordination are essential.

Qualifications

• 1 year of working in the Human Services field required.
• Strong knowledge of community resources, community services, and the legal/civil rights of seniors.

Responsibilities

• Conduct Home Delivered Meal (HDM) assessments and Expanded In-home services for the Elderly Program (EISEP) assessments, including in-home visits and NYSOFA forms.
• Ensure benefits for which a client is eligible are obtained and maintained per NYSOFA guidelines.
• Meet with clients to complete NYSOFA/OAA required forms and determine eligibility.
• Obtain annual diet prescription forms to verify HDM program appropriateness.
• Assess nutrition risk and refer to a registered dietician as needed.
• Maintain client files and update journals, handle events, and respond to changes in service needs.
