Job summary

Mary Free Bed Rehabilitation Hospital in Grand Rapids, MI seeks a Temporary Case Coordinator to support the Social Service Department for five months, starting in early December. The role is full-time, Monday–Friday 8am–5pm, and requires a degree in social work or a related field with healthcare experience.

You will assist in discharge planning, care coordination, and resident/family interviews, contributing to interdisciplinary teamwork and patient advocacy.

Qualifications

• Bachelor's or master's degree in social work or related field.
• One year of supervised social work experience in a healthcare setting.
• Experience in long-term care or geriatric population preferred.
• Supervisory experience preferred.
• Strong care coordination and discharge planning skills.

Responsibilities

• Assist in discharge planning and development of social care plans for residents.
• Interview residents/families to obtain social history and plan community services.
• Coordinate collaboration with interdisciplinary team and provide staff consultation.
• Enter information in Care Tracker and maintain documentation.
• Recruitment, interviewing, and selection of personnel for the Community or Department.
• Ensure charted notes reflect services and resident responses.
