Phaxis is seeking a Patient Care Coordinator in Charlotte, NC to organize physician orders, referrals, appointments, authorizations, and medical records with a patient-focused approach.

The role requires coordinating with physicians, handling EHR tasks, and ensuring timely communication across office locations in a healthcare setting.

Qualifications

• High school diploma required; some college preferred.
• Minimum 1 year of medical office or healthcare facility experience.
• EHR and scheduling experience required.
• Experience providing customer/patient service in person and over the phone.
• Strong organizational, communication, and attention-to-detail skills.

Responsibilities

• Schedule patient follow-up visits, ancillary appointments, procedures, and referrals
• Follow up on referrals and appointments to ensure physician orders are completed in a timely manner
• Complete forms and requisitions necessary to carry out physician orders
• Obtain and send medical records to physicians and outside facilities
• Obtain laboratory, X-ray, hospital, referral, and other ancillary results
• Verify insurance coverage and obtain/document authorization requirements for referrals and procedures
• Monitor assigned Athena/EHR inboxes and ensure documentation is handled appropriately
• Assist patients with questions regarding appointments, referrals, and access to care
• Maintain accurate and complete patient charts and documentation
• Communicate with physicians and healthcare professionals as needed
• Work at different office locations as needed

We are seeking an organized and patient-focused Patient Care Coordinator to join an established healthcare team in Charlotte, NC. This position plays an important role in ensuring physician orders, referrals, appointments, authorizations, and medical records are completed accurately and on time.

Pay: $20-$23/hour

Medical Office / Outpatient Healthcare Setting

Key Responsibilities
