Job description

What You Will Do to Change Lives

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and co-occurring disorders, assisting them to live as independently as possible within their community.

Shifts Available:

TEMP: 8:30 AM - 5:00 PM | Monday - Friday - May 20 - July 11

Expected starting wage range is $24.59 - $29.64. Telecare applies geographic differentials to its pay ranges based on the location of the role. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

  • High School Graduation or G.E.D. equivalent plus two (2) years of direct services experience in Mental Health or community services; or an Associate's degree with one (1) year of experience; or a Bachelor's degree in Social Services
  • Valid and current driver's license, with personal vehicle insurance in your name
  • Willingness to use your personal vehicle to drive clients to appointments and groups

Our teams collaborate closely with Kaiser's psychiatrists and psychologists to provide community-based services.
