Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and co-occurring disorders, assisting them to live as independently as possible in their community.
TEMP: 8:30 AM - 5:00 PM | Monday - Friday - May 20 - July 11th
Expected starting wage range is $24.59 - $29.64. Telecare applies geographic differentials to its pay ranges. The pay range will depend on the geographic location of the role. Starting pay is based on relevant experience above minimum requirements.

The Kaiser Intensive Community Treatment (ICT) program, part of Telecare, is an Assertive Community Treatment (ACT) program serving clients with Serious Mental Illness (SMI). It aims to support Kaiser members who are high users of inpatient psychiatric services or at high risk of hospitalization. Our teams work closely with Kaiser’s psychiatrists and psychologists to provide community-based services.
