Overview
We’re looking for a Telephonic Triage Nurseto join our team. You will directly report to the Telephonic Triage Manger. This is a remote position and you will be responsible to assess callers' symptoms, to prioritize calls and to advise patients or families in basic treatment or to make the critical decision to send a nurse to evaluate the patient’s current situation.
- Receives and responds to calls for assigned Hospice service area
- Collects appropriate data in order to make informed decisions related to patients care
- Prioritizes various patient care situations, providing information, education, support, and continuity of care in relation to needs identified on the Plan of Care to the patient’s family and/or significant others via the phone or assigns on-call RN or other appropriate discipline to complete an in-person patient visit

SAT 10p-8a | SUN 10p-8a | TUE 5p-9p |
THUR 5p-9p | FRI 5p-9p | SAT 10a-6:00p | SUN 10a-6:00p |
About You
- Graduate of an accredited school of nursing
- Current Registered Nurse (RN) licensure within a NLC Compact State,Three (3) years clinical experience
- Must possess a minimum of two (2) years’ experience, either in oncology, long-term care, or hospice or home health
- Prior experience with telephone triage
- Prior experience using Electronic Medical Records (EMR) and Microsoft Office Suit including Word and Outlook
