Responsibilities

• Document admission assessments, plans, interventions and medications per policy.
• Use the information system to communicate and track changes in patient condition.
• Communicate transfers of responsibility during bedside handoffs and rounds.
• Identify patient needs and refer related supports when appropriate.
• Incorporate pain management and supportive care into the care plan.
• Adhere to infection control standards and maintain a safe, clean environment.
• Perform other duties as assigned.
