Qualifications

Responsibilities

• Performs assessment for assigned patient across age-specific variables.
• Educates patient/family and assesses educational needs.
• Anticipates problems related to disease, discharge needs, and family support.
• Develops and updates nursing care plans with input from the interdisciplinary team.
• Coordinates communication with physicians and healthcare providers.
• Monitors patient response to treatment and initiates interventions.
• Prioritizes care as patient status changes.
• Participates in discharge planning and patient teaching.
• Adheres to Standards of Care and OSHA guidelines.
• Documents patient care accurately and timely.
• Engages patient/family in care planning.
• Coordinates care with other departments and community agencies.
