• Reports symptoms and changes in patients’ condition and vital signs; modify treatment plans as indicated.
• Consults with physicians and other healthcare professionals to assess, plan, implement and evaluate care plans.
• Prepares patients for examinations, procedures and treatments and assists as needed.
• Discharges planning, patient education and ensuring smooth transition of care.
