Qualifications

Responsibilities

• Assess physical, social, and psychological status and information from interdisciplinary team.
• Develop an individualized plan of care with patient, family and team involvement.
• Coordinate and delegate care to the interdisciplinary team to meet the plan of care.
• Evaluate patient response and revise the care plan as needed.
• Maintain accurate and timely documentation and uphold professional standards.
