Job summary

Responsibilities

• Performs hourly rounding and assessments per policy.
• Develops and revises a plan of care and teaching plan.
• Documents assessments and outcomes in Meditech on time.
• Coordinates teaching for discharge and family involvement.
• Communicates changes with physician using SBAR.
